ProActive Testing Solutions is a specimen collection provider. We perform the collection under the applicable chain-of-custody procedure and return the specimen to the designated laboratory. We do not perform laboratory analysis, issue or interpret results, or make or influence immigration or court determinations.

Please don't send client identification documents, case numbers, or dates of birth through this form. We don't need them to schedule a collection.
